EXPLICIT CONSENT TEXT ON THE PROCESSING AND PROTECTION OF PERSONAL DATA

As Cherry Jewellery, acting as the data controller, we process, store, and transfer your health data and certain personal data within the scope of the Law on the Protection of Personal Data (KVKK) and the General Data Protection Regulation (GDPR) due to reasons such as the performance of the contract, explicit provision by law, establishment, usage, or protection of a right, or legitimate interests of our company, provided that it does not harm your fundamental rights and freedoms.
Regarding your personal and special categories of data, you may submit your requests to exercise your rights such as obtaining information, deletion, destruction, correction, objection, and compensation for damages pursuant to Article 11 of the KVKK (and applicable GDPR articles) to the addresses listed below:

As Cherry Jewellery, offering jewelry and piercing services, we obtain your approval to process your identity data and health data for the stated purposes and reasons, and to transfer them to the recipient groups specified below. This approval will be realized with your consent.
Since piercing applications require explicit consent for the processing of your health and personal data in order to provide a healthy and safe service; we would like to state that if you do not provide your approval for the health and personal data requested in this form, the services to be offered (i.e., the performance of the contract) will not be possible. We need to process your identity, contact, and health data to provide you with better service and to maintain our contractual relationship.
I acknowledge that I have read and understood the Clarification Text on the Protection and Processing of Personal Data regarding the use of my personal and health data processed for the purposes stated above, limited to the processing purpose within the scope of the relevant process and contractual relationship, and their storage for the required period, and that the necessary clarification has been made to me during this process.
Based on the explanations above, as the data subject, I declare that I give my explicit consent to the processing of my personal data with my free will, having been informed.
